Children’s Mobile Response and Stabilization Services

  • Offers 24-7 crisis de-escalation, safety planning, and case management support to children and families in Monmouth County within one hour of their location.

Children’s Mobile Response and Stabilization Services (CMRSS) offer crisis de-escalation, safety planning, and case management support to children and families in Monmouth County.

Through the New Jersey Children’s System of Care, PerformCare connects young people and their families statewide to the care they need.

CPC assists with immediate crisis response, care management, and in-home services including:

  • Emotional and behavioral disorders
  • School attendance or behavioral concerns
  • Co-occurring mental health and substance use disorders
  • Intellectual/developmental disabilities​

Program for Assertive Community Treatment (PACT)

  • Community treatment for individuals with serious mental illness and substance use disorders using a team-based approach.  
  • PACT combines psychiatrists, nurses, clinical social workers, co-occurring specialists, vocational specialists, mental health advocates, and peer specialists. For more information call (732) 842-2001

PACT (Program for Assertive Community Treatment) is a community program that works with adults who have serious mental illness and co-occurring substance use disorders. PACT’s goal is to provide treatment so individuals can maintain their independence and remain in the community.

PACT uses a team-based approach: Each team includes a psychiatrist, a co-occurring disorders specialist, vocational specialists, nurses, mental health clinicians, mental health advocates, and peer specialists.

Supported Employment

CPC’s Supported Employment program (SE) assists individuals living with serious mental illness who are interested in entering or returning to the workforce.

SE offers vocational services, starting with clients identifying their career interests and preferences. SE staff work with clients to assess their unique strengths to find, attain, and maintain a job and career of their choosing.

SE offers on-the-job training to assist individuals interested in this service with learning their job duties and managing the employee/employer relationship. We provide SE based on each individual’s needs, including long-term assistance for those who need it. For more information call (732)-780-2012

Supportive Housing – Community Support Services

  • CPC offers Supportive Housing throughout Monmouth County. The scope and purpose of the program is to increase independence, community integration, and quality of life.

  • Completion of comprehensive rehabilitation needs assessment for consumer-directed treatment
  • Development of an individualized plan for recovery and wellness

Skills development training and skill teaching based on:

  • Daily living skills
  • Social skills

  • Budgeting, financial stability, and entitlements
  • Recreation and leisure
  • Self-advocacy
  • Wellness and a healthy lifestyle
  • Employment, volunteer, and educational readiness

Integrated Case Management Services

  • Supports adults with mental illness and/or co-occurring disorders to re-integrate into the community, providing linkages and resources.

Integrated Case Management Services (ICMS) assists adults with serious and persistent mental illness and/or co-occurring substance use disorders to increase independence and promote community integration.

Case management services provide linkages to help secure necessary services and resources while maintaining recovery and a high quality of life.

Participants can receive ICMS services for up to 12 months. ICMS works closely with other community healthcare providers, hospitals, and social service agencies.

Services include:

  • Individualized treatment services in the community for up to 12 months
  • Individual assessment and service planning
  • Linkage to community services and resources
  • Assistance in obtaining basic living necessities.
  • Coordination of care with family or support system

Justice Involved Services

The Justice Involved Services (JIS) program offers voluntary, community-based care management for adults diagnosed with serious and persistent mental illness and/or co-occurring substance use disorders who are involved in the criminal justice system. JIS focuses on assessing, planning, and coordinating services to support individuals throughout their legal involvement and to ensure a successful transition back into the community after incarceration.

Services include:​

  • Individualized Treatment: Tailored services to address specific needs, including special assistance related to parole or probation conditions.
  • Legal Support: Help in finding an attorney and providing advocacy during sentencing, along with individual assessment and service planning.
  • Community Linkage: Connecting individuals to essential community services and resources, include basic essential living necessities.
  • Care Coordination: Collaborating with the individual’s family or support system to ensure comprehensive care.

 

  • Health Provider Coordination: Linking and coordinating with other physical and behavioral health treatment providers.
  • In-Community Care Management: Crisis intervention and management provided within the community.
  • Supportive Counseling: Offering emotional and practical support through counseling.
  • Family Support: Providing psychoeducation and support to the individual’s family.
